Part-time Community Health Nurse – Rapid Recruitment

About the role

The Community Health Nurse will support the implementation of SAMS health projects, focusing on the Type 1 Diabetes Management initiative. This part‑time position involves direct beneficiary interaction, education, and coordination with local stakeholders to ensure effective service delivery.

Key responsibilities

• Support implementation of SAMS health programs, including the Type 1 Diabetes Management project.
• Identify, register and follow‑up beneficiaries eligible for project services.
• Distribute glucometers, test strips and other diabetes‑related supplies according to approved procedures.
• Provide basic education on glucometer use, diabetes self‑management and project services to beneficiaries and caregivers.
• Maintain regular communication with beneficiaries to ensure continuity of care and adherence to project requirements.
• Coordinate with community‑based organisations, health facilities and other stakeholders, supporting outreach and referrals.
• Collect, enter and verify beneficiary data in project databases, track indicators and prepare regular reports.
• Monitor inventory of glucometers and supplies, prepare purchase requests and support logistics coordination.

Required profile

• Qualified nurse with experience in community health or chronic disease programs.
• Understanding of Type 1 diabetes management and patient education.
• Strong organisational, communication and data‑management abilities.
• Ability to work independently in field settings and liaise with multiple stakeholders.

Required skills

• Glucose meter operation and diabetes self‑management tools.
• Data entry and basic database management.
